For Provider, Facility or Associate membership. This category conveys voting privileges and is required for Board of Directors.


Informational Membership - $300
For Facility or Associate membership who do not want to participate in active membership, but wish to be kept informed about legislative and public issues which affect the industry and who wish to participate in the seminar programs at the member rate.

Added Member - $100
For provider, Facility or Associate Full or Informational Members who wish to have an additional member of their organization receive mailings, and member alerts.
